I usually like it when something is linked to. Including a portfolio
as an attachment is quite cumbersome, and unless asked for
specifically, I would avoid it. However, linking to an online
portfolio is your best bet. It always gets my attention the quickest.

That pretty much depends on the company. When I was interviewing for
jobs, I spoke to everyone from strictly the HR person to the HR
person, the lead designer, and the Vice President of the company. It
all really depends.
Speaking for myself, I don't bother having resumes go through our HR
department first. I have them come straight to me. I like seeing the
resumes as soon as they come in, and looking through the online
portfolio's or reading about the person's experience.
