In World of Warcraft, at least since the last patch, you have large
regenerative abilities which shut down in combat.
While in combat, you don't regenerate a thing.
When out of combat, you regenerate "moderately fast" (and can speed
that up by eating or drinking, in which case you have to stop
moving). Players report extremely fast regen rates. Casters can
regenerate from 0 to full mana in less than 2mn, while moving to the
next spot.
