Lots of MUDs do that I believe. Achaea does, for instance. You can
target the head, left arm, right arm, torso, left leg, or right
leg. Similarly, there are various defences to help defend a particular
body part. A monk can concentrate on blocking and counter-throwing
people attacking a body part, and anyone with the parrying ability can
use a parry-capable weapon to parry a body part.

There are also various effects associated with a body part taking too
much damage. Concussions, bleeding, broken and mangled limbs, etc.

It hasn't really created any issues in a negative way.
