Design Patterns -- Elements of Reusable Object-Oriented Software by
Gamma, Helm, Johnson, and Vlissides (also know as the Gang of Four). 
Published by Addison-Wesley, ISBN 0-201-63361-2.

This book is a God.  Not even a minor diety, a full God.  Its in
something like its 10th printing now.  There is also a gang-of-four
mailing list where the book's contents and suggestions for Ver2 are
discussed.
